ABOUT THE WEBINAR
Case management must connect clients to the medical, social and community resources needed for optimal health. But payment gaps push case managers to work creatively and beyond. With CMS now agreeing to pay for some supportive services for those with chronic conditions, the landscape is shifting, but it will take culture change across the continuum to get there. What role will case managers play?
After the webinar, participants will be able to:
- Identify systemic barriers faced by case managers and community-based service providers;
- Describe efforts to build evidence-based case management practices in order to influence reimbursement streams; and
- Understand new reimbursement and contracting opportunities that open doors for case management services.
