Engaging patients and their family as collaborators in their care is a cornerstone of the national strategy for delivering health care that results in better care, better health and lower costs. But, by and large, it continues to be an aspirational aim rather than business as usual. This webinar will focus on creating a framework for consistent, sustainable patient- and family-engaged care.
Learning Objectives:
- Identify the role of patients and families in engagement and the foundational principles of participatory engagement;
- Discuss the benefits for patients (and their caregivers) when they gain access to their own records and become engaged in their ongoing care; and
- Describe three ways case managers can invite engagement from patients and family members.
