Bonnie Ewald is the manager of strategic development and policy for the department of social work and community health at Rush University Medical Center in Chicago. In this position, she leads several program development and policy advocacy initiatives related to integrating health care and social services at Rush and across the country, including managing the Center for Health and Social Care Integration, which serves as a national convening and technical assistance hub to advance practices that break down barriers to health. Bonnie earned a master’s degree in public policy studies from the University of Chicago, a post-graduate certificate in sustainable urban design from Archeworks and a bachelor’s degree in mathematics and geography from the University of Wisconsin.

ABOUT THE WEBINAR

Case management must connect clients to the medical, social and community resources needed for optimal health. But payment gaps push case managers to work creatively and beyond. With CMS now agreeing to pay for some supportive services for those with chronic conditions, the landscape is shifting, but it will take culture change across the continuum to get there. What role will case managers play? 
 
After the webinar, participants will be able to:

  • Identify systemic barriers faced by case managers and community-based service providers;
  • Describe efforts to build evidence-based case management practices in order to influence reimbursement streams; and
  • Understand new reimbursement and contracting opportunities that open doors for case management services.

This webinar is pre-approved by the Commission to provide one CCM CE credit to board-certified case managers. CCM CE credits are only available to those who attend the live webinar. CCM CE credits are not available by viewing the webinar after it airs. The cost of CCM CE credits is $25 and must be paid in advance.

The Commission for Case Manager Certification is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

Each person is required to register individually to claim CCM CE credit. Groups viewing the webinar together cannot claim CE credit.

Individuals will be sent an evaluation after the webinar. Once you complete your evaluation, you will be able to download a certificate of attendance if you paid for CE credit, and your dashboard will be updated.
If you miss the webinar and you have registered for CE credit, no refund will be issued.
