CCMC kicks off 2016-2017 CMLearning Network free webinar series with focus on Triple Aim and hospital case management

MOUNT LAUREL, N.J., Sept. 1, 2016—Health care has too often been sickness care, focused more on episodes and procedures than on the patient. But that is gradually changing, driven by new reimbursement models that value quality over volume and an understanding that the patient, not the system, must be at the heart of health care delivery.

This change is inspired by the Institute for Healthcare Improvement’s Triple Aim, a focused effort to improve the patient experience of care (including quality and satisfaction); improve the health of populations; and reduce the per capita cost of health care. The Triple Aim, and how it relates to hospital case management, is the focus of the first in the upcoming CMLearning Network® webinar series hosted by the Commission for Case Manager Certification®.

In the Sept. 20 webinar, Janet Tomcavage, RN, MSN, chief population health officer, Geisinger Health System, will discuss the Triple Aim’s role in improving the health of populations. Tomcavage will examine strategies and tools to assess and manage population health in order to achieve better outcomes, improved patient experience and lower costs. “It makes sense that we launch the CMLearning Network series by focusing on the Triple Aim,” said Commission CEO Patrice Sminkey. “From coordinating care to serving as patient advocates and leaders on the health care team, the professional case manager’s work is about ensuring the right care, delivered in the right place at the right time. That essentially parallels the Triple Aim. I’m especially eager to hear Janet’s insights on how Geisinger accomplishes this in the hospital setting.”

The Commission hosts its CMLearning Network webinar series to educate and raise awareness of current issues and solutions for case managers and others involved in coordinated, multi-disciplinary care. Routinely, more than 500 professional case managers sign up for each event to learn about topics such as care coordination, care transitions, ethical issues and regulatory changes that affect case managers. 

From September 2016 through May 2017, the Commission will host five complimentary webinars that support professional development. The first, featuring Tomcavage, is set for Sept. 20, 2016, from 1 to 2 p.m. Eastern. It’s free to participants, although board-certified case managers may register to earn continuing education credit for a small fee. 

November’s featured webinar speaker, Allison Sampson Jackson, PhD, LCSW, LICSW, CSOTP, will discuss assessment for social determinants of health, focusing on the CDC’s Adverse Childhood Experiences question set. Other topics in the 2016-2017 series will include new payment models, behavioral health integration and ethics in today’s case management practice.
